Kindergarten: 4+ DTap;/DTp/ D/Td; 3+ Polio, 2MMR, 3 Hep. B, 1 Vaucella or proof of disease.

• All Students must present documentation of month, day and year of each immunization before they attend school.

• All immunizations must be up-to-date before students are permitted to attend class.

• Immunizations may be obtained from your physicians office, or at your local health department. Make sure to take your childs immunization record with you.
